5. Detailed copy or screenshot of valid CA Single Subject: Dance OR valid CA Single Subject: Physical Education Credential. Please do not include the credential certificate copy (*See below on how to obtain your detailed credential)
6. Copy of: EL Authorization (if included on your credential, copy of credential will be sufficient.)
7. Copy of: Transcripts

3. Enter your "Social Security Number and Birth Date"
4. Select your valid teaching credential for this position and print.
