Master's Degree or higher in Speech-Language Pathology
Valid California Clinical or Rehabilitative Services Credential or Speech Language Pathology Services Credential ;Valid California Drivers License; Valid First Aide and CPR Certificate; Valid Non-Violent Crisis Intervention Certificate

• Copy of Transcript
• CPR/First Aid Certification
• Letter of Introduction
• Letter(s) of Recommendation (minimum 3 letters )
• Resume

• Speech-Language Pathology Services Credential - Language, Speech and Hearing (or Proof of enrollment in an SLP Master’s program (official enrollment verification or letter))
